EEC requires every child enrolled to have an annual physical exam and a record of all immunizations on file. If we do not have up-to-date medical information for your child, they cannot attend First Circle. One to two months before your child’s physical has expired, we will send you a reminder. Parents have within 30 days of their previous annual physical date to turn in a copy of a current physical form. Once 45 days have passed after a physical has expired, a child cannot attend First Circle. We have blank health record forms, although most physicians use their own. Your child’s doctor may mail or fax the record directly to us [see PROGRAM-SPECIFIC INFORMATION], or you may bring in a copy. If you bring in the original, we can copy it.
